In conjunction with Primary Care Network Kirkby, we are looking to recruit two Social Prescribing Link Workers. Full details of the GP Practices covered can be found here.
Social prescribing empowers people to take control of their health and wellbeing through referral to non-medical ‘link workers’ who give time, focus on ‘what matters to me’ and take a holistic approach, connecting people to community groups and statutory services for practical and emotional support. Link workers support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners.
Initially the role will assist in providing support to individuals deemed to be at the highest risk, in response to the current incident with COVID- 19 and to mobilise community networks to support those same individuals.
The closing date for applications is midday on the 15th January 2021 . For further details please click the Job Description and Person Specification.
